چکیده

P aget disease is a common condition characterized by increased bone turnover and abnormal bone deposition in the skull and vertebral column. This article describes a previously undiagnosed case of Paget disease in which the manifesting clinical symptom was gradually progressive diplopia from sixthnerve compression due to skull base hyperostosis. The sixth-nerve palsy, which was severe at the initial examination, slowly resolved after the patient was treated with intravenous zoledronic acid. To my knowledge, Paget disease has not previously been reported to manifest initially with diplopia, nor has diplopia due to this condition been reported to improve with medical therapy.

Sixth Nerve Palsy from Cholesterol Granuloma of the Petrous Apex

Herein, we report a patient who had an isolated sixth nerve palsy due to a petrous apex cholesterol granuloma. The sixth nerve palsy appeared acutely and then spontaneously resolved over several months, initially suggesting a microvascular origin of the palsy. Subsequent recurrences of the palsy indicated a different pathophysiologic etiology and MRI revealed the lesion at the petrous apex. Sur...

Isolated sixth nerve palsy

An isolated sixth nerve palsy may be a harbinger of underlying intracranial disease. Due to its long subarachnoid course, it may be damaged by downward shift of the brainstem as often occurs in increased or decreased intracranial pressure (“false-localizing sixth nerve palsy”). Alternatively, the sixth nerve may be involved in isolation by a compressive lesion in the cavernous sinus or along th...

Acute Otitis Media-Induced Gradenigo Syndrome, a Dramatic Response to Intravenous Antibiotic

Introduction: Petrositis is a rare but severe complication of acute otitis media and mastoiditis. Despite efficient antibiotic therapy, there are still reports of both intratemporal and intracranial complications of otitis media with the potential risk of high morbidity and mortality. Petrositis has traditionally been treated with surgery, but recent advances in imaging, with improved antibioti...

Persistent effect of zoledronic acid in Paget's disease.

Paget's bone disease is a disorder in which bone regions with high turnover are replaced by new, vascular, but disorganized and immature bone with excessive fibrosis, high tendency of deformity and diminished mechanical resistance. Treatment aims at the suppression of osteoclast activity and is achieved with bisphosphonates, which represent the treatment of choice for Paget's disease. Zoledroni...

Multiple Myeloma Manifesting as a Fluctuating Sixth Nerve Palsy

We report a case of multiple myeloma that presented as a fluctuating sixth cranial nerve palsy in the absence of widespread signs of systemic disease. A 63-year-old woman presented with horizontal diplopia of two weeks duration that subjectively changed over time. Ocular examination showed a fluctuating sixth nerve palsy. A computed tomography (CT) scan of the brain showed multiple, enhancing, ...
